The medication is taken on an as-needed basis prior to anticipated sexual activity. It is not an aphrodisiac and requires direct sexual stimulation to be effective. Its use is common among men whose ED has vasculogenic, neurogenic, or psychogenic origins.

Dosage and direction

The recommended starting dose of Zhewitra Soft is one 20mg tablet, taken approximately 15-60 minutes before sexual activity. Place the tablet on the tongue; it will disintegrate rapidly within seconds and should be swallowed with the saliva. Do not take with water. The dosing frequency should not exceed once in a 24-hour period. The dosage may be adjusted based on efficacy and tolerability under medical supervision. It can be taken with or without food, though consumption with a high-fat meal may delay the time until onset of effect.

Precautions

• Prior to use, a thorough medical evaluation is recommended to diagnose the underlying cause of ED and assess cardiovascular health.

Possible side effect

Like all medicines, Zhewitra Soft can cause side effects, although not everybody gets them. Most are mild to moderate and transient.

• Very common (≥1/10): Headache.
• Common (≥1/100 to <1/10): Flushing, dizziness, nasal congestion, dyspepsia (indigestion).
• Uncommon (≥1/1,000 to <1/100): Blurred vision, changes in color vision, photophobia, palpitations, tachycardia, nausea.
• Rare (≥1/10,000 to <1/1,000): Hypotension, syncope, priapism (a prolonged and painful erection lasting more than 4 hours).
• Frequency not known: Hearing loss (sudden), non-arteritic anterior ischemic optic neuropathy (NAION).

Drug interaction

Vardenafil is primarily metabolized by the cytochrome P450 enzyme CYP3A4. Concomitant use with inhibitors or inducers of this enzyme can significantly alter its plasma concentration.

• Contraindicated: Organic nitrates (e.g., nitroglycerin), nitric oxide donors, guanylate cyclase stimulators (riociguat).
• Not recommended: Potent CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, ritonavir, indinavir, clarithromycin). Concomitant use requires significant dose reduction and medical supervision.
• Use with caution: Moderate CYP3A4 inhibitors (e.g., erythromycin, saquinavir, amlodipine), alpha-blockers (e.g., doxazosin, tamsulosin) due to additive blood-pressure-lowering effects, other antihypertensive agents.
• Potential interaction: Grapefruit juice may increase Vardenafil plasma concentrations and should be avoided.

Missed dose

Zhewitra Soft is not intended for regular daily dosing but is taken on an as-needed basis. Therefore, the concept of a “missed dose” does not apply. Simply take one tablet when you plan to engage in sexual activity, ensuring at least 24 hours have passed since any previous dose.

Overdose

In the event of an overdose, the expected effects would be an exaggeration of the known dose-related adverse effects, such as severe headache, profound hypotension, syncope, and prolonged erection. Standard supportive measures should be applied. As Vardenafil is highly protein-bound, dialysis is not expected to accelerate clearance. In cases of priapism, immediate medical attention is required to avoid permanent tissue damage and impotence. Treatment of severe hypotension may require active cardiovascular support, including fluid administration.
