Common use

Dosage and direction
For as-needed treatment of erectile dysfunction, the recommended starting dose is 10 mg, taken orally at least 30 minutes prior to anticipated sexual activity. The dose may be increased to 20 mg or decreased to 5 mg based on efficacy and tolerability. Dosing should be limited to a maximum of one dose per 24-hour period. For daily use, the recommended dose is 2.5 mg, taken orally at approximately the same time each day, without regard to the timing of sexual activity. The daily dose may be increased to 5 mg based on individual efficacy and tolerability. Tadasiva may be taken with or without food. The tablet should be swallowed whole with a glass of water; it should not be split, crushed, or chewed.
Precautions
Prior to prescribing Tadasiva, physicians should conduct a thorough medical and sexual history assessment to determine the underlying etiology of ED and identify appropriate treatment candidates. Patients with cardiovascular disease should be evaluated for their suitability for sexual activity, as it involves a certain degree of cardiac exertion. Use with caution in patients with anatomical deformation of the penis (e.g., angulation, cavernosal fibrosis, or Peyronie’s disease) or conditions that may predispose them to priapism (e.g., sickle cell anemia, multiple myeloma, or leukemia). Patients should be advised that Tadasiva offers no protection against sexually transmitted diseases. Prolonged erections lasting more than 4 hours require immediate medical attention to prevent long-term tissue damage.
Contraindications
Tadasiva is strictly contraindicated in all patients who are using any form of organic nitrates or nitric oxide donors (e.g., amyl nitrite “poppers”), either regularly or intermittently, due to the risk of a precipitous and potentially fatal drop in blood pressure. It is also contraindicated in patients with a known hypersensitivity to Tadalafil or any of the tablet’s excipients. Concomitant use with guanylate cyclase stimulators, such as riociguat, is contraindicated. Its use is not recommended in patients with severe hepatic impairment (Child-Pugh Class C) or severe uncontrolled hypertension.
Possible side effect
The most commonly reported adverse reactions are headache, dyspepsia, back pain, myalgia, nasal congestion, flushing, and limb pain. These are typically transient and mild to moderate in severity. Back pain and myalgia are often described as a deep muscular discomfort and usually resolve within 48 hours. Less common side effects include dizziness, visual disturbances (non-arteritic anterior ischemic optic neuropathy has been reported rarely), tinnitus, and palpitations. Priapism (a painful erection lasting more than 4 hours) is a rare but serious adverse event requiring urgent medical intervention.
Drug interaction
Concomitant administration with nitrates is absolutely contraindicated. Potent inhibitors of the cytochrome P450 3A4 (CYP3A4) isoenzyme (e.g., ketoconazole, itraconazole, ritonavir, clarithromycin) can increase Tadalafil exposure; a dose reduction is recommended. Concomitant use with alpha-adrenergic blockers (e.g., doxazosin) can potentially lead to symptomatic hypotension; such combination therapy should be initiated only if the patient is stable on their alpha-blocker therapy. Tadasiva may potentiate the hypotensive effects of antihypertensive medications. Alcohol can potentiate both the blood-pressure-lowering effects and orthostatic signs and symptoms.
Missed dose
For the as-needed dosing regimen: This regimen is not dependent on a daily schedule. If a dose is missed, it is not to be replaced; the next dose should be taken at least 24 hours before the next anticipated sexual activity. For the once-daily dosing regimen: If a dose is missed, the patient should take it as soon as remembered. If it is almost time for the next dose, the missed dose should be skipped, and the regular dosing schedule resumed. The patient should not take a double dose to make up for the missed one.
Overdose
In cases of suspected overdose, standard supportive measures should be employed. The primary expected hemodynamic effect is hypotension. Cardiovascular status should be monitored and appropriate management for hypotension instituted, which may include fluid resuscitation and, if necessary, vasopressor agents. Hemodialysis is not expected to accelerate clearance as Tadalafil is highly bound to plasma proteins and not significantly eliminated in the urine.
Storage
Store at room temperature between 20°C to 25°C (68°F to 77°F), with excursions permitted between 15°C and 30°C (59°F and 86°F). Keep the medication in its original blister pack or container to protect it from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
