Common use

Silagra is primarily indicated for the treatment of erectile dysfunction in adult males. It is used on an as-needed basis to improve erectile function. It is not intended for use by women, children, or adolescents. It does not protect against sexually transmitted infections and is not a form of contraception.

Dosage and direction

The recommended starting dose for most patients is 50mg taken approximately one hour before anticipated sexual activity. Based on efficacy and tolerability, the dose may be increased to a maximum of 100mg or decreased to 25mg. Dosage adjustment is necessary for patients with hepatic impairment, severe renal impairment, or those taking certain concomitant medications (e.g., potent CYP3A4 inhibitors). The tablet should be swallowed whole with a glass of water. Do not take more than one dose in a 24-hour period.

Precautions

Contraindications

It is also contraindicated in patients taking guanylate cyclase stimulators, such as riociguat.

Possible side effects

The most commonly reported adverse reactions are headache, flushing, dyspepsia, nasal congestion, and dizziness. These are generally mild to moderate in intensity and transient. Visual disturbances (e.g., changes in color perception, blurred vision) have been reported. Less frequently, patients may experience back pain, myalgia, or nausea. Priapism (a prolonged and painful erection lasting more than 4 hours) is a rare but serious adverse event requiring immediate medical intervention to prevent permanent tissue damage. Sudden decrease or loss of hearing or vision has been reported in temporal association with PDE5 inhibitor use. Discontinue use and seek prompt medical attention if any of these occur.

Drug interaction

Concomitant use with nitrates is contraindicated. Potent inhibitors of the cytochrome P450 3A4 pathway (e.g., ketoconazole, itraconazole, ritonavir) can significantly increase sildenafil plasma concentrations, necessitating dose adjustment. Alpha-blockers may potentiate the blood-pressure-lowering effects. Grapefruit juice may increase plasma levels of sildenafil and should be avoided. Sildenafil may potentiate the hypotensive effects of alcohol.

Missed dose

Silagra is not intended for chronic daily dosing. It is taken on an as-needed basis approximately one hour before sexual activity. Therefore, the concept of a “missed dose” does not apply. Do not take an extra dose to make up for a missed occasion.

Overdose

In cases of suspected overdose, standard supportive measures should be employed. Given its vasodilatory effects, the primary manifestation is likely to be hypotension. Management of hypotension may include placing the patient in the Trendelenburg position and administering intravenous fluids. Dialysis is not expected to accelerate clearance as sildenafil is highly protein-bound.

Storage

Store at room temperature (15Β°C to 30Β°C) in the original blister package to protect from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
