Dosage and direction
The recommended starting dose for most patients is 50 mg taken approximately one hour prior to anticipated sexual activity. Concomitant use with guanylate cyclase stimulators, such as riociguat, is also contraindicated.
Possible side effect
The most commonly reported adverse reactions include headache, flushing, dyspepsia, nasal congestion, and dizziness. Drug interaction

Alpha-blockers and antihypertensive agents may potentiate the hypotensive effects. Concurrent administration with other PDE5 inhibitors is not recommended. Use with recreational drugs containing alkyl nitrites (“poppers”) is contraindicated. Grapefruit juice may increase Sildenafil exposure and should be avoided.
Missed dose
As Nizagara is taken on an as-needed basis, the concept of a “missed dose” does not apply. It should only be taken when planned sexual activity is anticipated. Patients should not take an extra dose to make up for a missed opportunity, as exceeding the maximum recommended frequency (once per day) increases the risk of adverse effects.
Overdose
In cases of overdose, standard supportive measures should be employed. Given the vasodilatory properties of Sildenafil, hypotension could be expected. Management may include placing the patient in the supine position with legs elevated and administering intravenous fluids if necessary. Hemodialysis is not expected to significantly accelerate clearance, as Sildenafil is highly protein-bound and extensively metabolized.
Storage
